Care Visions at Home provide care and support for people with advanced or progressive complex needs and/or life limiting conditions within their own home environment. Our aim is to provide personal centre outcome based support that will enable our customers to maintain their independence.We pride ourselves on offering a supported living service that puts our customers at the centre of their care. Care Visions At Home provide support via Local Authority contracts, Continuing Healthcare, CCG's, Direct Payment, Independent Living Funds, Personalised Health Budget holders and for those customers who wish to pay for private support such as assistance with shopping, companionship, live in care, respite to name but a few. Whether you require additional support on a short, mid or long term basis we are here to help our customers every step of the way.We also offer a professional and dedicated service to children and young people who have special needs which range from social care to more complex clinical requirements. We implement and carefully manage the care needs of our customers taking into consideration their individual emotional, physical & intellectual development as the work towards a positive adulthood. Our main aim is to provide quality care & support ensuring that customers achieve their full potential by offering a bespoke outcome led service that goes above & beyond traditional domiciliary care.

End of Life, Palliative Care, Complex Care, Mental Health, Learning Disabilities, ABI, PSD, Spinal Injuries, Children, Young People, Families
We have fully trained branch nurses to manage our packages of care that require nurse led intervention.
