ABOUT LOCAL CARE PROVIDER - LIVING AT HOME SERVICE
Living at Home Service is a reablement service which provides people in their own homes with short term reablement support following discharge from hospital after a stroke, orthopaedic trauma or accident. The team has dedicated physiotherapy and occupational therapy staff attached to aid with rehabilitation. Support can be offered between two and six weeks.
