ABOUT LOCAL CARE PROVIDER - AVENUES LONDON (WEST KENT)
The Community Support Services Team provides support to people living in their own homes through three different services. The Young People's service supports young people and their families. The outreach service supports adults with a range of needs including learning disability, mental health needs, older people and those with physical disabilities. The supported living service provides 24 hour support to people with learning disabilities.
