ABOUT LOCAL CARE PROVIDER - THE REABLEMENT TEAM
The Reablement team offers short-term intensive support to enable people to regain their independence. This is usually following an accident or hospital admission. All referrals to the service are via the local care management teams. At any given time there are approximately 40 people using the service.
